Proton Care Navigator

Overview:

Covenant Health (Tennessee) is seeking a skilled Proton Care Navigator to join our team at the Thompson Proton Center. As a key member of our clinical staff, you will play a vital role in ensuring the seamless coordination of care for patients who have completed treatment and are returning for follow-up care.

Responsibilities:

  • Schedule follow-up appointments and imaging as indicated in the patient's survivorship follow-up plan.
  • Provide guidance, solutions, and assistance to patients and their families, serving as a primary resource of information for follow-up patients.
  • Ensure a smooth and cohesive operation for follow-up patients, maintaining a positive culture throughout their ongoing care.
  • Effectively communicate with referring providers, clinical staff, multidisciplinary colleagues, patients, and other parties.
  • Review clinic schedules to fill all available appointment slots accurately and in accordance with established clinic-specific guidelines.
  • Prepare and provide patient instructions and appointment details orally and in writing, as defined by department guidelines.
  • Proactively engage patients to identify opportunities for them to benefit from survivorship events, public tours, and other relevant activities.
  • Demonstrate effective and professional communication skills and call handling, including diffusing upset/distressed customers.
  • Demonstrate knowledge of medical terminology and follow policies, procedures, and safety standards.

Qualifications:

We are seeking an individual with a minimum of two to three years of experience in a medical setting or high-volume customer call center. A bachelor's degree in a directly-related field from an accredited college or university is preferred.
