Job Title: School-Based Licensed Practical Nurse (LPN)
Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Soliant in a part-time school setting. As a School-Based LPN, you will play a vital role in providing high-quality healthcare services to students, ensuring their health and well-being in a supportive and inclusive environment.
Responsibilities:
- Healthcare Services: Assess and respond to students' health concerns, injuries, and illnesses promptly, providing appropriate care and referrals as needed.
- Individualized Healthcare Plans: Develop and implement individualized healthcare plans for students with specific medical needs, in collaboration with parents/guardians and other healthcare providers.
- Health Records: Maintain accurate and confidential health records, including immunization records, health assessments, and documentation of healthcare services provided.
- Health Education: Educate students, parents, and staff on health-related topics, including preventive care, hygiene practices, and emergency procedures.
- Accommodations: Provide accommodations for students with disabilities as needed per school's requirements.
- Transportation: Assist students with transportation to and from school as needed.
Qualifications:
- LPN License: Valid LPN license in CT.
- Experience: Experience working as a nurse in a school, pediatric, or related healthcare setting (experience in an educational setting preferred).
- Disability Accommodations: Experience with disability accommodations (i.e. G-tube and ventilator assistance) preferred.
- Clinical Skills: Strong clinical skills, including first aid, medication administration, and assessment of common childhood illnesses and injuries.
- Communication Skills: Excellent communication and interpersonal skills to interact effectively with students, parents, and staff members.
- Teamwork: Ability to work independently and collaboratively as part of a multidisciplinary team.
