Job Summary
We are seeking a skilled and compassionate Primary Care Physician to join our team of healthcare professionals. As a Primary Care Physician with HarmonyCares, you will have the opportunity to provide personalized care to elderly individuals and those with complex medical issues, utilizing proven care models to deliver exceptional patient outcomes.
Responsibilities
- Deliver primary health care and patient care management in a residential setting
- Provide primary care to high acuity patients in their home, Monday - Friday
- Utilize mobile diagnostic services and state-of-the-art technology to deliver high-quality care
- Collaborate with our team of healthcare professionals to ensure seamless patient care
Qualifications
- Experience in Geriatrics or complex patients preferred
- Active/unrestricted medical license and DEA
- Board Certified or Board Eligible
- EMR experience
