Key Responsibilities:- Maintain regular contact with clients, Treatment Foster Parents, and biological/adoptive parents to ensure effective support and coordination.
- Coordinate the activities of the treatment team, providing liaison and advocacy with community-based services and supports as needed.
- Develop and implement treatment plans in collaboration with the treatment team, ensuring alignment with client needs and goals.
- A master's degree in social work or a related field, or a bachelor's degree in a related field with two years of experience working with this population.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to maintain confidentiality and handle sensitive information with discretion.
