Job Summary:

The MDS Nurse plays a pivotal role in ensuring the delivery of quality, cost-effective care to residents. This individual manages the process of assessment, planning, facilitation, coordination, evaluation, and advocacy for services and options to meet the unique needs of each resident.

In this position, you will be responsible for managing the RAI Process from resident admission to discharge, maintaining clinical compliance, and receiving appropriate funding from Medicare, Medicaid, and Managed Care pay sources. Your attention to detail and ability to develop accurate, timely, and case mix-optimized MDS forms are crucial in this role.

You will work closely with the interdisciplinary team to develop individualized care plans that reflect the resident's status. Additionally, you will coordinate resident assessments and services by other clinical departments, provide timely information for managed care clinical authorization, and manage daily PPS/OBRA case management meetings and weekly level of care meetings.

We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience.
