Opportunity Overview

We are seeking a skilled Hospice Care Nurse Specialist to join our team, responsible for evaluating patients for hospice appropriateness, participating in the information gathering and analysis aspects of the admissions process, and building effective relationships with referral sources and internal stakeholders.

About the Role
This position requires strong analytical and communication skills, as well as the ability to work effectively in a fast-paced environment. You will have the opportunity to work closely with patients and their families, ensuring seamless transitions to care and providing compassionate support throughout the admissions process.

Key Responsibilities
- Evaluate patients for hospice appropriateness and facilitate admission orders
- Participate in the information gathering and analysis aspects of the admissions process
- Build effective relationships with referral sources and internal stakeholders
- Utilize interpersonal skills to educate patients and their families about hospice and palliative care

Requirements
- Previous experience in hospital, physician practice, LTC or ALF settings
- Bachelor's degree preferred
- Strong analytical and communication skills
