Job Summary:

We are seeking a skilled LVN Case Manager to support the delivery of direct patient care in our mental health programs. The successful candidate will work within a multidisciplinary team to provide medical coordination, assessment, and accompaniment to clients diagnosed with mental illness and co-occurring conditions.

Key Responsibilities:

  1. Conduct routine patient care functions as an LVN, including medication management, treatment compliance, and skill building for memory and self-sufficiency.
  2. Administer medications by injection as determined by the treating provider, checking prescribed dosages against specified limits and observing and reporting adverse reactions as necessary.
  3. Use and practice basic assessment, patient care, and document pertinent patient information and data in accordance with JWCH policies and procedures and the standards of LVN practice.
  4. Work within a multidisciplinary team to support clients diagnosed with mental illness and co-occurring conditions through medical coordination, assessment, and accompaniment.
  5. Develop patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
  6. Provide consultation to and coordinate care for patients needing specialty mental health and/or substance use disorder services.
  7. Maintain standards of cleanliness, organization, and an adequate level of supplies for the assigned work area.
  8. Participate in assigned staff and other meetings, including quality improvement, case conferences, and in-service meetings.
  9. Conduct screenings for all service requests in a timely manner, as defined by DMH Access to Care standards.
  10. Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation.
  11. Participate in structured and as-needed case conferences.
  12. Assist with provider education for integrated behavioral and primary care services.
  13. Provide ongoing case management services, including linkage and referral to both internal programs and external resources.
  14. Support clients in building skills to reduce impairment from medical and mental health illnesses.
  15. Provide field-based care as deemed necessary for client treatment.
  16. Perform other duties as assigned.

Requirements:

  1. Current and valid California LVN License
  2. Current BLS card from the American Heart Association.
  3. At least one-year experience in mental health setting or working with mentally ill population.
  4. Ability to communicate effectively orally and in writing.
  5. Experience in working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds.
  6. Current and valid California Driver's License with current car insurance and a clean driving record.
  7. Bilingual (English/Spanish) is preferred.
