Job Summary

The SSVF Healthcare Navigator is a critical role in assisting Veterans who are homeless or at-risk of homelessness to end their housing crisis. As a member of the Volunteers of America Mid-States team, you will provide services that enhance the Veteran's independent living skills, connect them with community resources, and empower them to maintain long-term housing stability and self-sufficiency.

Key Responsibilities
  • Conduct assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others.
  • Provide case management duties, including meeting and setting up appointments with the Veteran and treatment team through virtual means/telehealth.
  • Act as a health coach by proactively supporting the Veteran to optimize treatment interventions and outcomes.
  • Establish linkages with appropriate agencies and service providers in the area/community.
  • Provide referrals and resources to support the Veteran's needs.
Requirements
  • A Master of Social Work or a Master's degree in a related field and less than five (5) years of work experience in the field.
  • Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
  • Ability to work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.
