As a Nurse Practitioner on our HouseCalls team, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their own homes. This proactive approach helps identify and mitigate health risks, ensuring patients receive the care they need to thrive.

Key Responsibilities:

  • Conduct comprehensive in-home assessments, including medical history reviews, medication reconciliation, and physical exams
  • Perform evidence-based practice screenings, such as point-of-care testing, as needed
  • Develop and implement personalized care plans to address patient needs and improve health outcomes
  • Communicate findings and recommendations to patients' primary care physicians to ensure seamless care coordination
  • Identify and respond to urgent and emergent situations, providing timely interventions as needed
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices

Requirements:

  • Active NP license and ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
  • Ability to complete physical demands of the job, including lifting, navigating stairs, and working in various dwelling conditions
  • Access to reliable transportation for travel to patient homes (up to 85% of the time)
  • Strong communication and interpersonal skills, with a patient-centered approach

Preferred Qualifications:

  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
  • Home health care or home visit experience
  • Additional licensure in other geographic areas (as needed)
