Position Overview:

The role of the Clinical Care Manager is essential in delivering high-quality nursing services aligned with physician directives. This position emphasizes the coordination of patient care to enhance health outcomes and operational efficiency. It necessitates advanced nursing knowledge and skills developed through both education and practical experience.

Key Responsibilities:

  • Utilize case management methodologies to guarantee comprehensive and synchronized patient care.
  • Conduct thorough clinical evaluations and formulate individualized care plans centered around patient needs.
  • Engage patients effectively through motivational interviewing techniques to foster better health management.
  • Oversee and improve care quality for vulnerable populations.

Qualifications:

  • Completion of an accredited nursing program.
  • Possession of a valid Registered Nursing (RN) license.
  • A minimum of three years of clinical experience in an acute care environment.
  • Valid driver's license required.

Preferred Qualifications:

  • Bachelor's degree in nursing or related field.
  • Five years of experience in acute, subacute, or home health care settings.
  • Certification in Case Management.
  • Bilingual capabilities are a plus.

Duties:

  • Prepare for and facilitate patient interactions.
  • Coordinate all facets of patient care effectively.
  • Develop and implement patient-centered care strategies.
  • Serve as a pivotal member of interdisciplinary patient care teams.
  • Conduct patient assessments and home visits as necessary.
  • Maintain clear communication with healthcare providers and team members.
  • Engage in quality improvement initiatives.

Physical Requirements:

  • Work in a professional office setting.
  • Primarily sedentary role with occasional movement.
  • Ability to communicate effectively and exchange information.
  • Visual acuity to read computer screens and documents.
  • Occasional lifting or moving of items up to 25 pounds.

  • Familiarity with community resources.
  • Exceptional interpersonal communication skills.
  • Ability to collaborate with diverse populations.
  • Understanding of chronic health conditions and their management.
  • Experience with motivational interviewing practices.
  • Strong organizational and multitasking capabilities.

  • Experience with Electronic Medical Records systems.
  • Ability to work autonomously as well as part of a collaborative team.
