Job Summary

We are seeking a skilled Case Manager to join our team at Boston Health Care for the Homeless Program. As a Case Manager, you will play a critical role in providing comprehensive case management services to homeless individuals, including benefits, housing, primary and other health care, and social and psychiatric services.

Key Responsibilities
  • Participate as a member of a multidisciplinary care team to provide comprehensive case management services, including outreach to individuals experiencing homelessness.
  • Follow patients through the entire course of treatment, providing education, service coordination, and linkage to benefits, housing, health care, social and behavioral health services.
  • Participate in ongoing quality improvement endeavors, identifying and care planning for high-risk patients, and facilitating continuity of care.
  • Provide outreach to clients as needed to facilitate comprehensive case management services.
  • Participate in regional provider meetings and attend clinic staff meetings and team meetings.
Qualifications
  • 1-2 years of experience working with individuals experiencing homelessness and knowledge of the network of services available to individuals experiencing homelessness.
  • BA/BS preferred, with a degree in behavioral health or social services.
  • Excellent verbal and written English communication skills, with problem-solving skills.
  • Bilingual in Spanish/English strongly preferred, with additional compensation for demonstrated verbal proficiency.
