About the Role:

Lifting Up Westchester is seeking a highly skilled Housing Support Coordinator to join our team. This dynamic individual will be responsible for assisting individuals in shelters find affordable housing and providing ongoing support to ensure their stability and independence.

Key Responsibilities:

  • Identify and manage landlord relationships to secure housing opportunities for clients.
  • Assist clients in finding and transitioning into suitable housing, including completing paperwork and coordinating with landlords.
  • Develop pre- and post-housing plans with clients, ensuring they have the necessary skills and resources to maintain their new living arrangements.
  • Provide ongoing support and advocacy for clients, addressing any barriers that may threaten their self-sufficiency.
  • Maintain accurate records of client interactions and progress.

Requirements:

  • Bachelor's degree in social work or related field (strongly preferred).
  • Minimum 4 years of experience in case management or housing placement for homeless populations.
  • Excellent communication and interpersonal skills, with the ability to form strong relationships with clients and landlords.
  • Basic computer skills, including data entry and Microsoft Office.
  • Valid driver's license required.
