About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Home Health team at Adoration Home Health and Hospice. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver individualized care plans.

Responsibilities
  • Provide safe and timely patient care using evidence-based nursing practices.
  • Participate in weekly team meetings to coordinate patient care and manage COVID-19 safety measures.
  • Develop and implement individualized treatment plans to facilitate patient rehabilitation and efficient outcomes.
  • Provide medication instruction, monitoring, and administration as ordered by the patient's Primary Care Physician.
  • Conduct patient assessments and develop care plans with interdisciplinary team members.
  • Supervise Licensed Practical Nurses and Home Health Aide staff as needed.
  • Document patient care and communicate changes in patient needs to the physician and nursing supervisor.
Requirements
  • Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state.
  • One year nursing practice in a patient care setting, with home health, geriatrics, or other relatable settings preferred.
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
  • Current CPR certification.
