Job Title: Patient Care Assistant

Department: Medical/Surgical

Reports To: Nursing Director/Charge Nurse

Position Overview: Deliver personalized care to residents while fostering a supportive physical and emotional environment under the guidance of a licensed nurse. Patient Care Assistants report directly to the charge nurse.

Qualifications:

  • Must possess a valid nursing assistant certification
  • Must maintain good standing with the relevant state authority

Key Responsibilities:

  • Adhere to attendance and punctuality standards
  • Follow the dress code; casual attire such as t-shirts, tank tops, cut-offs, open-toed shoes, or jeans (except on designated days) are not permitted.
  • Participate in mandatory monthly departmental meetings and all-staff training sessions
  • Treat residents, their families, and colleagues with dignity and respect
  • Complete required training by specified deadlines (e.g., CareLearning courses)
  • Abide by established Standards of Behavior
  • Follow organizational policies and procedures
  • Demonstrate independence, flexibility, integrity, and the ability to collaborate effectively with residents, staff, and support services
  • Communicate with residents and staff in a courteous and respectful manner
  • Collaborate as a team member while maintaining a positive outlook
  • Ensure confidentiality of resident information and uphold residents' privacy rights
  • Share resident information appropriately with the charge nurse and during shift changes
  • Treat residents with respect and dignity at all times
  • Strictly adhere to residents' rights
  • Assist in personalizing Activities of Daily Living (ADLs) for each resident
  • Support fellow CNAs even when not assigned to a specific area
  • Be adaptable to changing work areas as needed for the facility's best interests
  • Utilize creativity and propose new ideas for improved care methods
  • Accurately document tasks performed
  • Assist residents in attending scheduled activities
  • Help residents with appointments and transfers as necessary
  • Maintain the ADL documentation system for assigned areas
  • Provide or assist with scheduled bathing and oral care for residents
  • Dress residents appropriately for weather conditions and personal preferences
  • Offer and assist with hair care
  • Provide back rubs as needed
  • Administer appropriate skin care under the supervision of the charge nurse
  • Ensure residents are repositioned according to their care plans
  • Ensure clean linens are provided weekly and as needed
  • Measure and record vital signs as per care plans in coordination with the charge nurse
  • Provide or assist with toileting and incontinence care according to care plans
  • Report any changes in residents' elimination patterns to the charge nurse promptly
  • Ensure residents receive adequate hydration and nutrition throughout the shift
  • Assist with meals in dining areas as required
  • Engage in Quality Assurance initiatives
