Job Title: NHCH Caregiver

Nuvance Health is seeking a compassionate and skilled NHCH Caregiver to join our team. As a Caregiver, you will provide essential support to clients in their homes, assisting with daily living activities and promoting independence.

Responsibilities:
  • Assist clients with personal care activities, including bathing, dressing, and feeding.
  • Support clients with exercises, ambulation, and transfer activities, using devices such as canes, walkers, and lifts.
  • Prepare and serve meals, adhering to special diets and dietary restrictions.
  • Remind clients to take their medications and assist with medication management.
  • Monitor client condition and report any changes to the care team.
  • Provide basic homemaking and laundry assistance, maintaining a safe and orderly environment.
  • Transport clients to appointments and errands.
  • Fulfill all compliance responsibilities related to the position.
Requirements:
  • High School Graduate or Equivalent
  • Certified Nursing Assistant or graduate of a CT state-approved nurse aide training program
  • Valid Driver's License and satisfactory driving record
Working Conditions:

This role requires significant manual skills, motor coordination, and finger dexterity. You will work in clients' homes, exposing you to dirt, odor, noise, and human waste. You will exert up to 50 lbs. of force and work in a fast-paced environment.
