Position Overview:
Under the supervision of a registered nurse (RN), the Licensed Practical Nurse (LPN) delivers nursing care in alignment with hospital policies and procedures, as well as adhering to the State Practice Act. The LPN collaborates with the RN in assessing, planning, executing, and evaluating patient care. This role involves meticulous documentation of care provided, including interventions, outcomes, and patient/family education. The LPN works closely with physicians and other healthcare professionals to ensure seamless continuity of care. Additionally, the LPN is accountable for directing and assigning nursing tasks that may be performed by others, as well as executing care and interventions delegated by the RN.

Key Responsibilities:
  1. Patient Care Adaptation: Adjusts care approaches to meet the diverse needs of various age groups, including adolescents, adults, and geriatric patients.
    • Incorporates cultural considerations into care delivery.
    • Demonstrates knowledge of developmental stages.
    • Engages family members in decision-making processes.
  2. Nursing Process Competence: Assists the RN in prioritizing and adapting the interdisciplinary care plan to ensure quality service delivery.
    • Conducts delegated parts of the admission assessment as per unit standards.
    • Updates and documents the patient's care plan established by the RN.
  3. Implementation of Care: Exhibits the skills and judgment necessary to carry out the medical care plan and nursing interventions in collaboration with the RN.
    • Maintains knowledge of medication administration protocols.
    • Functions effectively in emergency situations.
  4. Documentation: Ensures timely and accurate documentation of care provided to facilitate quality assurance and information sharing among the healthcare team.
  5. Department Responsibilities: Executes all duties as required by the Acute Rehabilitation Department, addressing the unique needs of each patient.
    • Initiates appropriate nursing actions in crisis situations.
  6. Additional Duties: Demonstrates flexibility in meeting the unit's scheduling needs and participates in shared leadership initiatives.

Job Requirements:
Education: Graduate of an accredited nursing program required.
Licenses and Certifications: Current LPN license from the State of Nebraska required. Basic Life Support (BLS) certification must be obtained within one month of hire. Crisis Prevention Institute (CPI) training is required within six months of hire.
Experience: Previous clinical experience is preferred.
Skills: Proficiency in reading, writing, speaking, and understanding the English language is required.
