Position Overview:

The Physician will play a crucial role within the medical team at the Department of Veteran Affairs, focusing on the Geriatrics Rehabilitation and Extended Care Service (GREC) patient care line, Home Based Primary Care (HBPC), and Hospital in Home (HiH) programs. This role requires a physician with the capability to provide a comprehensive range of patient care services, emphasizing both primary and geriatric health care.

Key Responsibilities:

  • Delivering direct patient care or overseeing fellows/residents in both home and outpatient environments.
  • Collaborating with hospital and Ambulatory Care personnel to facilitate seamless referrals and care for veterans receiving home care.
  • Performing thorough assessments, diagnosing, and managing various medical, geriatric, and palliative health issues.
  • Initiating and modifying treatment plans, ordering necessary tests, consultations, and medications in accordance with VA guidelines.
  • Providing clinical insights for patient management strategies within the HBPC and HiH programs.
  • Assisting with other Geriatrics and extended care initiatives as required.
