Key Responsibilities
  • Evaluate barriers to care and engage patients and families in developing potential solutions, taking into account their physical and emotional well-being, literacy, cultural background, and information needs.
  • Identify and coordinate internal and external resources to address patient needs, continuously expanding knowledge of community resources, including immigration, employment, and financial assistance.
  • Educate patients and caregivers about the multidisciplinary nature of cancer care and survivorship, clarifying the roles of team members and what to expect from the healthcare system.
  • Empower patients by fostering self-management skills and providing information about available resources and referrals.
  • Facilitate effective communication between patients, families, and healthcare providers to optimize patient outcomes.
  • Assess patients' ability to advocate for themselves and assist them in prioritizing questions and clarifying information during their interactions with the care team.
  • Contribute to the development, implementation, and evaluation of the patient navigation program; follow through on special projects and tasks as assigned by management.
  • Demonstrate empathy and provide information and support within the scope of practice; refer to licensed clinicians as necessary.
  • Adhere to organizational policies and procedures to ensure compliance with HIPAA, privacy, safety, and other regulations; document care provided in accordance with Fred Hutch policies.

Qualifications

Required:

  • High school diploma.
  • A minimum of three years of experience in social services, healthcare, or a related field.
  • Strong communication skills, with the ability to interact effectively and courteously with patients and families from diverse cultural backgrounds.
  • Ability to collaborate effectively with a multidisciplinary team.
  • Excellent organizational skills and proficiency in both written and oral communication.
  • Capacity to work independently, manage multiple tasks, and prioritize effectively.
  • Demonstrated reliability and timely communication as a team player.
  • Fluency in English, both oral and written.
  • Familiarity with community resources in the greater Puget Sound Region and beyond.
  • Proficient in Microsoft Office and other relevant software applications.

Preferred:

  • Bachelor's degree.
  • Fluency in a language other than English is advantageous.

Compensation and Benefits
The hourly pay range for this position is competitive and will be determined based on experience and qualifications.
