Care Coordination Specialist

We are seeking a highly skilled Care Coordination Specialist to join our team at Memorial Healthcare System. In this role, you will be responsible for coordinating the transfer and placement of patients across our healthcare system, ensuring seamless and efficient care delivery.
Key Responsibilities:
- Coordinate patient transfers and bed placements across Memorial Healthcare System facilities
- Communicate effectively with healthcare teams, including surgery, emergency department, nursing units, and physician offices
- Monitor and document transfer needs, ensuring timely and accurate patient placement
- Collaborate with interdisciplinary teams to coordinate patient care and optimize outcomes
- Develop and implement effective plans of care to meet patient needs and maximize available resources
Requirements:
- Bachelor's degree in Nursing (Required)
- Minimum of three (3) years of Emergency Department or Critical Care experience
- Registered Nurse License (RN LICENSE) - State of Florida (FL)
- BLS Cert American Heart_non ecard (BLS AHA) - American Heart Association (AMERICAN HEART)
