Job Summary:
We are seeking a highly skilled Registered Nurse to join our team as a Care Coordination Specialist. In this role, you will be responsible for coordinating the transfer and placement of patients within our healthcare system.
Key Responsibilities:
- Coordinate patient transfers and bed placements across Memorial Healthcare System facilities
- Communicate effectively with surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure timely and efficient patient transfer and placement
- Monitor and document transfer needs, communicate patient condition to sending or receiving providers and facilities
- Consult with other disciplines to determine patient needs, such as infection control, equipment needs, nutritional, functional, pain, social, and skin integrity
- Serve as an expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervene as needed
- Follow established protocols for interfacility transfer acceptance and escalate to leadership as necessary
- Document transfer and bed placement in the Epic EMR to ensure adequate patient safety standards are met
- Collaborate with all disciplines to coordinate patient transfers, including disposition, ambulance transportation, specialist referrals, and bed assignments
- Anticipate admission needs and communicate findings with transferring departments, nurses, or facilities to optimize patient flow
- Plan, implement, and evaluate patient needs based on reported assessment to optimize outcomes and maximize available resources
- Utilize critical care or emergency department experience to identify patient care needs and provide expert recommendations for correct facility transfer
Requirements:
- Bachelor's degree in Nursing
- Minimum of three (3) years of Emergency Department or Critical Care experience
- American Heart Association (BLS) certification
- Registered Nurse License (RN) in the State of Florida
Working Conditions:
This role requires critical thinking skills, effective communication, decisive judgment, and the ability to build and foster positive relationships. The incumbent must be able to lead others and take appropriate action when required. Can adjust through a rapidly changing environment.
