We are seeking a highly skilled RN to join our Care Coordination Center team at Memorial Healthcare System. As a Care Coordination Specialist, you will play a critical role in ensuring the smooth transfer and placement of patients across our healthcare system.
Key Responsibilities- Coordinate patient transfers and bed placements across Memorial Healthcare System
- Communicate effectively with surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure timely and efficient patient transfer
- Monitor and document transfer needs, communicate patient condition to sending or receiving providers and facilities
- Consult with other disciplines based on patient needs, including infection control, equipment needs, nutritional, functional, pain, social, and skin integrity
- Serve as an expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervene as needed
- Follow established protocols for interfacility transfer acceptance and escalate to leadership as required
- Document transfer and bed placement in the Epic EMR to ensure adequate patient safety standards
- Collaborate across all disciplines to coordinate patient transfers, including disposition, ambulance transportation, specialist referrals, and more
- Bachelor's degree in Nursing (Required)
- Minimum of three (3) years of Emergency Department or Critical Care experience
- Registered Nurse License (RN LICENSE) - State of Florida (FL)
- BLS Cert American Heart Association (AMERICAN HEART)
