Job Summary

We are seeking a highly skilled and compassionate Enrollment Coordinator to join our team at Brio Living Services. As the first point of contact for potential PACE members and their families, you will play a critical role in educating and guiding them through the enrollment process.

Key Responsibilities
  • Conduct face-to-face functional assessments at potential members' homes or in acute care settings to identify critical care needs.
  • Collaborate with the potential member, their support system, and the PACE care team to create a transitional care plan.
  • Assist potential members in applying for long-term care Medicaid and facilitate conversations to help them seamlessly transition into the PACE program.
  • Maintain and submit records, including enrollment paperwork, progress notes, and Medicaid documentation.
  • Link individuals to and collaborate with appropriate community agencies and resources.
  • Consult with and advise team members on the relationship of social, emotional, and cultural factors to health and medical care, and the availability of services in the community.
  • Assist with community engagement and outreach efforts.
  • Participate in quality improvement activities to ensure the highest level of care for our members.
Requirements
  • Active State of Michigan Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW) license.
  • Valid driver's license and reliable vehicle for travel (mileage will be reimbursed).
  • Minimum of one year of experience working with the frail elderly or long-term care population.
