Job Summary:
As a Community Care Worker I with the Texas Department of Aging & Disability Services, you will play a vital role in determining the needs and eligibility of individuals for Community Care services. Your work will involve conducting home visits, interviewing clients, and documenting information to develop service plans that meet their needs. You will also verify data, explain program benefits and requirements, and refer clients to appropriate services. Additionally, you will authorize and monitor CCAD services to ensure they are meeting the individual's needs.

Key Responsibilities:

• Conduct home visits and interviews to assess client needs and provide information and referral to other resources.

• Review client eligibility and functional needs to determine the best course of action.

• Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.

• Maintain accurate and up-to-date case records and enter information into an automated system.

• Monitor client services to ensure they are meeting their needs and make adjustments as necessary.

• Develop and coordinate service plans with clients, families, and other stakeholders to ensure effective service delivery.

Requirements:

• Ability to establish and maintain effective relationships with clients, colleagues, and other stakeholders.

• Strong communication and interpersonal skills to effectively interact with clients and colleagues.

• Ability to prioritize tasks, set deadlines, and meet expectations in a fast-paced environment.

• Knowledge of interviewing techniques and resources that serve individuals with disabilities.

• Ability to operate computer and general office equipment.
