Care Coordinator I

As a Care Coordinator I with the Texas Department of Aging & Disability Services, you will play a vital role in determining the needs and eligibility of individuals for Community Care for the Aged and Disabled (CCAD) services. Your work will involve conducting home visits, interviewing individuals, documenting information, and developing service plans to meet their needs. You will also verify data, explain program benefits and requirements, refer individuals for appropriate services, and ensure authorizations are registered in automated systems. Key Responsibilities:
•Attend work on a regular and predictable schedule and perform other duties as assigned.
•Conduct home visits and interviews to assess the need for services and provide information and referral to other resources.
•Review and determine eligibility and functional needs for CCAD services.
•Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs.
•Maintain accurate and up-to-date case records and enter information in an automated system.
•Monitor services to ensure they meet the individual's needs and develop service plans accordingly.
•Prepare reports and communicate with stakeholders to ensure effective service delivery. Requirements:
•Ability to establish and maintain effective relationships with individuals, coworkers, and other stakeholders.
•Ability to prioritize tasks, set deadlines, and meet expectations.
•Knowledge of interviewing techniques, resources, and service delivery systems.
•Ability to operate computer and general office equipment.
•Strong communication and interpersonal skills.
