Key Responsibilities:- Provide personal care and homemaking services, including light housekeeping, meal planning, and preparation, and assisting with activities of daily living.
- Assist patients with mobility, transfers, and ambulation, using assistive devices as needed.
- Respond to the physical, mental, social, spiritual, cultural, and emotional needs of patients and their families.
- Chart patient information and communicate changes in patient status to the healthcare team.
- Current Certification as a Certified Home Health Aide.
- Valid Driver's License.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
The work environment is typically moderate, with occasional exposure to noise and stress. You must be able to safely lift and/or roll clients, and utilize a Hoyer lift and other FDA mechanical lifts as needed.
