Job Overview

At Creative Change Counseling, we are seeking a highly skilled and compassionate Licensed Intensive In-Home and Community Therapist to join our team.

Key Responsibilities:

  • Conduct initial screenings and mental health assessments to identify service needs and recommend options for care.
  • Develop and implement individualized treatment plans that address the unique needs and goals of each client.
  • Provide individual, family, and group therapy services, as well as case management and coordination of care.
  • Collaborate with clients, families, and community agencies to ensure seamless delivery of services and achieve optimal outcomes.
  • Monitor and evaluate treatment progress, making adjustments as needed to ensure the best possible results.

Requirements:

  • Licensed in the State of New Jersey (LCSW, LPC, LMFT, LSW, LAC)
  • A master's degree in a Mental Health field (e.g., Social Work, Psychology, Counseling)
  • Three (3) years of mental health experience, with a focus on intensive in-home and community services
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong assessment and treatment planning skills, with the ability to adapt to changing circumstances
  • Proficiency in Windows and Microsoft Office software, with the ability to learn and utilize new technology as needed

Working Conditions:

This position requires intermittent sitting and standing, as well as constant use of hands for computer keyboard and report writing. Occasional lifting of up to 101bs may be required. Good visual and auditory acuity are necessary for monitoring and responding to group participation.
