The Position
We are seeking part-time and full-time licensed mental health counselors (LMHCs) to work with the adult and geriatric population in a Skilled Nursing and/or Assisted Living setting. As a psychotherapist with Vitae, you will have the opportunity to work with a dedicated patient base and a consistent referral flow.
Key Responsibilities
- Provide psychotherapy services to patients in a Skilled Nursing and/or Assisted Living setting
- Collaborate with the skilled nursing facility's integrated care team and Vitae's clinical team
- Develop and implement individualized treatment plans for patients
- Conduct regular assessments and evaluations to monitor patient progress
Requirements
- Hold an active and unrestricted LMHC license in the state
- Completed master's degree in a related field
