Job Summary

Patriot Hospice is seeking a compassionate and skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality patient care and coordinating services to meet the unique needs of our patients and their families.

Key Responsibilities

  • Coordinate and provide hospice care services to patients and their families, ensuring a seamless and supportive experience.
  • Develop and implement individualized care plans, working closely with the interdisciplinary team to address the physical, emotional, and spiritual needs of patients.
  • Conduct thorough assessments of patients' needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with healthcare providers, family members, and caregivers to ensure comprehensive care and support.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and company policies.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements

  • Current RN licensure in the State of South Carolina.
  • 1 year of experience in a hospital, long-term care facility, or related healthcare setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care and service.
  • Knowledge of hospice care principles, practices, and regulations, with a commitment to delivering high-quality care and support.

Working Conditions

  • Requires travel within the agency's service area, with occasional overnight stays.
  • Works in office areas, community settings, and long-term care facilities, with exposure to a variety of environments and situations.
  • May be subject to physical demands, including lifting, bending, and standing for extended periods.
