Job Title: MDS Coordinator

Charlotte Health & Rehabilitation Center is seeking a dedicated and skilled MDS Coordinator to join our team. As a Registered Nurse, you will be responsible for completing minimum data set assessments and creating comprehensive plans of care for our patients.

Key Responsibilities:
  • Conduct thorough clinical assessments of patients to identify their needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care delivery.
  • Develop and implement effective care plans that meet the unique needs of each patient.
  • Ensure compliance with federal and state regulations, as well as company policies and procedures.
Requirements:
  • RN license
  • Experience in skilled nursing and rehabilitation environment
  • Strong clinical skills and knowledge of MDS 3.0 process
  • Excellent communication and interpersonal skills
