As a Primary Care Physician - Senior Health Specialist, you will be responsible for providing high-quality, patient-centered care to our seniors, addressing their physical, emotional, and social needs. You will work closely with our interdisciplinary care team to develop and implement personalized care plans, ensuring that our patients receive the best possible outcomes.

Responsibilities
As a Primary Care Physician - Senior Health Specialist, your key responsibilities will include:
- Evaluating and treating patients in accordance with standards of care.
- Developing and implementing personalized care plans in collaboration with the care team.
- Providing education and support to patients and their families on disease management and health promotion.
- Actively participating in daily huddles and care team meetings to ensure seamless communication and coordination of care.
- Maintaining accurate and timely medical records, including chart reviews and data analysis.
You will also have the opportunity to participate in quality improvement initiatives, contribute to the development of new services and programs, and engage in ongoing education and professional development to stay current with best practices in senior care.
Requirements
To be considered for this role, you must have:
- A current and unrestricted medical license in the state of Florida or ability to obtain a medical license in the state of practice.
- Graduation from an accredited MD or DO program.
- Excellent verbal and written communication skills.
- A high level of skill in interpersonal relationships and communications with colleagues and patients.
- Fully engaged in the concept of 'Integrated team-based care' model.
- Willingness and ability to learn and adapt to practice in a value-based care setting.
- Superior patient/customer service skills.
- Basic computer skills, including email and EMR.
Preferred Qualifications
We also welcome applications from candidates with:
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active and unrestricted DEA license.
- Medicare Provider Number and Medicaid Provider Number.
- Minimum of two to five years directly applicable experience.
- Experience managing Medicare Advantage panels and understanding of Best Practice in coordinated care environments.
- Knowledge of Medicare guidelines and coverage.
- Bilingual with Spanish a plus.
- Knowledge of HEDIS quality indicators.

Salary Range
The estimated annual salary for this role is $224,400 - $275,000, based on experience and qualifications. This is a competitive range, and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, and performance.
This role is eligible for a bonus incentive plan, based on company and/or individual performance.
