Job SummaryWe are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Hospice RN/ RN Case Manager. As a key member of our interdisciplinary team, you will provide direct patient care services, support patients and their families, and collaborate with healthcare professionals to deliver high-quality care.
Responsibilities- Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and evaluating patient outcomes.
- Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
