About the Role:

We are seeking a skilled Nurse Practitioner to join our Clinical team. As an Advanced Care Provider, you will conduct comprehensive health assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and health plans we serve. Your primary responsibility will be to perform detailed evaluations of the medical and social assessment of beneficiary members.

Responsibilities:

- Complete comprehensive assessments face-to-face in-home.
- Identify and refer members/patients for routine, urgent, and emergent case management.
- Make recommendations for member's follow-up with their primary care physician.
- Not required to prescribe or alter the member's current treatment plans.

Requirements:

The ideal candidate should have 2 years of hands-on experience as a Nurse Practitioner, must be registered with Medicare and Medicaid, and hold a current, unrestricted license in the state of assignment. Additionally, the candidate should possess knowledge of the HCC model, HEDIS, and STAR quality measures. Strong communication skills are necessary to effectively interact with elderly and chronically ill members.

Benefits:

We offer competitive compensation of $100 per completed visit. The position is based in Vancouver, Washington, and surrounding areas, requiring reliable transportation to travel to patients' homes. We also provide flexible scheduling and part-time positions available.
