Coordinate Care and ServicesCoordinate care with external providers and ensure accurate documentation in our Health Information Systems.
Advocate for ClientsAdvocate for clients' access to necessary benefits and services, including Medicaid and third-party insurance.
Engage in Community OutreachEngage in community outreach to build strong, culturally sensitive relationships with clients and their families.
Requirements- Oklahoma licensed LPN or RN with experience in community-based mental health.
- Fluency in Spanish, with strong verbal and written communication skills.
- Ability to build rapport with clients, colleagues, and external partners.
- Demonstrated commitment to recovery-focused, trauma-informed care.
- Must meet all certification and training requirements, including Therapeutic Options and First Aid/CPR.
