LPN Communicable Disease Specialist

We are seeking a skilled LPN to join our team at Variety Care, specializing in communicable disease management. As a key member of our clinical practice, you will play a vital role in providing exceptional patient care and support.
Job Summary:The LPN Communicable Disease Specialist will be responsible for coordinating patient care, conducting health assessments, and administering medications and injections. You will work closely with our healthcare team to ensure seamless care delivery and maintain accurate records in our electronic medical records system.
Key Responsibilities:- Conduct health assessments, record patient vital signs, and provide routine care as directed by providers.
- Coordinate patient care, including referrals, prior authorizations, and scheduling.
- Administer medications, injections, and vaccines, ensuring accurate documentation and adherence to safety protocols.
- Maintain accurate records in our electronic medical records system, ensuring timely and secure data entry.
- Communicate effectively with patients, providers, and staff to ensure seamless care delivery and address patient concerns.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in communicable disease management.
- Active LPN license in the state of Oklahoma.
- Five years of clinical work experience, with a focus on communicable disease management.
- Basic computer skills, including data entry and typing.
- Ability to obtain and record accurate information in electronic medical records software.
- Bilingual (English/Spanish) or proficiency in a second language.
- Completion of CTR training offered by OSDH.
- ACLPN - AIDS certified licensed practical nurse.
- Immunization state training.
