Job Summary:

Sigma Healthcare Team is seeking an Occupational Therapist to join our team. As an Occupational Therapist, you will work onsite in our client's location, providing client-related therapy services. Shift: Monday - Friday 8 am-5pm

Key Responsibilities:
  • Perform initial and ongoing client assessments and apply appropriate goals and interventions to resolve identified problems.
  • Provide Occupational therapy services to clients according to physician orders and regulations.
  • Assure physicians are notified in a timely manner of changes in client status and physician's orders are obtained as needed for changes to the plan of care.
  • Identify and obtain/make appropriate modifications to equipment/environment to meet client needs.
  • Provide teaching and instruction to client/caregiver to promote physical and emotional health.
  • Attend required staff meetings and case conferences.
  • Contribute to the coordination of services through effective and timely communication with other members of the health care team.
  • Maintain the caseload and agency productivity standards.
  • Respond to referrals to Occupational Therapy in a timely manner and document care according to the agency standards.
  • Maintain clients' self-esteem, privacy, and confidentiality of personal information.
  • Assure client rights are protected and that all clients/caregivers are made aware of the client's rights and responsibilities.
  • Initiate emergency procedures as appropriate.
  • Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.
  • Perform other duties within scope of practice, as assigned by Sr. Director of Clinical Operations.
  • May require extended staff schedules.
Requirements:
  • Graduate of an Accredited School of Occupational Therapy.
  • Maintains NBCOT Membership Requirements.
  • Currently licensed and in good standing in the State of Colorado.
  • Previous home care experience and Occupational Therapy experience preferred.
  • Valid driver's license and auto insurance, ability to drive to and from client homes.
Computer Skills:
  • Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.).
  • Basic Internet navigation skills.
