Job Summary:

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to patients in their homes. As a member of our team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized treatment plans that promote physical and emotional well-being.

Responsibilities:
  • Conduct initial and ongoing assessments to identify client needs and develop effective treatment plans.
  • Provide occupational therapy services to clients in their homes, including exercises, activities, and education to promote independence and mobility.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Document client progress and maintain accurate records.
  • Participate in staff meetings and case conferences to share knowledge and best practices.
Requirements:
  • Graduate of an Accredited School of Occupational Therapy.
  • Maintains NBCOT Membership Requirements.
  • Currently licensed and in good standing in the State of Colorado.
  • Previous home care experience and Occupational Therapy experience preferred.
  • Valid driver's license and auto insurance, ability to drive to and from client homes.
  • BLS.
