Key Responsibilities- Assist in completing needs assessments and care plans to ensure seamless transitions for our guests.
- Support in facilitating discharge processes and locating housing for individuals in need.
- Contribute to the completion of discharge notes and dissemination to hospitals to ensure continuity of care.
- Collaborate with Social Services Coordinators, Social Services Manager, and nursing department to execute weekly case conferencing for guests.
- Conduct daily check-ins with guests to ensure their needs are met and provide emotional support.
- Intermittently conduct random room searches and comply with harm reduction procedures to ensure a safe environment.
- Conduct warm hand-offs for guests when appropriate to ensure a smooth transition.
- Bachelor's degree preferred or minimum of one (1) year relevant experience in healthcare delivery, hospital education, social services, or similar field.
- Excellent communication skills, both oral and written, to effectively present information and respond to questions from directors, case managers, and discharge planners.
- Positive and professional attitude, with a strong commitment to providing exceptional care and support to our guests.
- Time management and strong organizational skills to prioritize tasks and meet deadlines.
- Computer proficiency in MS Word, Excel, Outlook, PowerPoint, and proficient typing skills to maintain accurate records and reports.
- Frequent standing and walking, as well as extended sitting, to provide care and support to our guests.
- Frequent bending, stooping, squatting, reaching above the shoulders, twisting, and turning to navigate our facilities.
- Frequent lifting, pulling, pushing, and moving up to 50 pounds to ensure a safe and accessible environment.
- Must utilize conversational speech for effective verbal communication with guests and team members.
- Noise level is high in some client care areas, requiring adaptability and flexibility.
