Job Overview

We are seeking a highly skilled and compassionate Case Manager Specialist to join our team at Urban Strategies LLC. In this role, you will be responsible for managing an assigned caseload of post-release services, collaborating with care providers, and assessing the prospective sponsor's ability to provide a safe living environment for children.

About the Role

As a Case Manager Specialist, you will have the opportunity to work with diverse populations, utilize trauma-focused approaches, and brief therapeutic techniques to support children and their sponsors. You will also maintain appropriate documentation, safeguard confidential records, and protect personally identifiable information according to program regulations.

Responsibilities
  • Manage an assigned caseload of post-release services
  • Collaborate with care providers in verifying and assessing additional information
  • Evaluate the prospective sponsor's ability to provide a safe living environment
  • Assess, identify, verify, and make recommendations for areas where additional support or resources may be needed
  • Maintain telephone contact and conduct virtual and in-person home visits
  • Utilize trauma-focused approaches and brief therapeutic techniques to support children and their sponsors
  • Maintain appropriate documentation and safeguard confidential records

Requirements
  • Bachelor's degree in Social Work or related field
  • At least 1 year experience managing cases
  • Skills in writing professional assessment reports and working with immigrant populations
  • Knowledge of family preservation, kinship care, and trauma-informed practices
  • Effective communication and interpersonal skills

Estimated Salary
$74,067 per annum.
