About the Role

We are seeking a highly skilled and dedicated PSH Case Manager - Long-term Housing Specialist to join our team. As a key member of our housing support services team, you will play a critical role in assisting individuals to obtain and maintain stable housing.

About the Job
  • Assist in identifying and engaging eligible program participants when there is a vacancy.
  • Assist prospective program participants in filling out the intake referral applications and obtaining necessary supporting documentation.
  • Coordinate with the Housing Locator to identify available affordable units and request housing locations and inspections as required.
  • Assist program participants with securing and filling out leasing applications and assist with moving into their unit once approved.
  • Schedule home visits and coordinate linkages to available County and other mainstream services, including transportation to and from appointments as needed.
  • Maintain accurate and complete weekly case notes and participant files, including release of information, up-to-date case notes, weekly service plans, and individual service plans.
Responsibilities
  • Complete rental calculations annually or as needed and deliver rent checks at the end of each month as required.
  • Input accurate and complete data timely in the Efforts to Outcomes client database and report on program participant successes and challenges during bi-weekly Housing Team Meetings.
  • Act as an on-call contact person for property management companies/landlords and attend landlord liaison meetings quarterly.
  • Communicate effectively with other service providers and conduct treatment team meetings as needed to provide continuity of care for program participants.
  • Work as part of a team to share information, strategies, mutual support, and ideas to address long-term needs of homeless individuals.
Requirements
  • Bachelor's degree in a related field (BSW) or equivalent experience.
  • Experience working with substance abuse, mental health issues, and chronically homeless individuals is preferred.
  • Bi-lingual Spanish/English a plus.
  • Valid driver's license and clean driving record.

This is a full-time position with benefits and salary range of $45,000-$60,000 depending on experience. We offer flexible work arrangements and opportunities for professional growth and development.
