Job Summary

We are seeking a skilled and compassionate Hospice LPN to join our team as a Patient Care Specialist. In this role, you will be responsible for providing direct patient care, collaborating with healthcare teams, and ensuring the highest level of quality and safety in patient care.

Key Responsibilities

  • Deliver high-quality patient care, adhering to established standards and protocols.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Document patient care activities, progress, and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care.
  • Maintain accurate and up-to-date patient records, adhering to confidentiality and data protection policies.
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless care coordination and collaboration.

Requirements

  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Oklahoma.
  • Previous experience in hospice care or a related field, with a strong understanding of palliative care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
