The Hospice Licensed Practical Nurse (LPN) is responsible for the provision of nursing care to patients according to an established physician plan of care, with service delivery under the direction of a registered nurse and in compliance with applicable state Nurse Practice Act regulations and agency policy and procedure. 

Qualifications

Currently licensed to practice as a Licensed Practical Nurse in the State of Oklahoma. Previous hospice nursing experience preferred.  Certification in Hospice and Palliative Nursing is preferred but not required. Proficient in verbal, written, and electronic communication. Must have valid driver's license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions. Must be able to maintain concentration during frequent interruptions. 

Responsibilities  

Delivers visits/service to assigned patients and performs direct care in accordance with physician-ordered plan of care to provide optimal care.  Assists Case Manager in ongoing observation and care of assigned patients.  Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation. Documents all nursing observations, interventions and evaluations in a concise, accurate and timely manner.  Reports patient-related observations to Case Manager daily and as needed.  Participates in on-call, holiday, and weekend rotation as assigned.  Participates in evaluation of patients in preparation for discharge.  Uses supplies appropriately and demonstrates a cost-conscious awareness by practicing careful, efficient and non-wasteful use of supplies.  Maintains equipment and supplies in proper functioning order and reports any malfunction immediately.  Demonstrates willingness to assist co-workers whenever needed.  Maintains a good working rapport with all patient care personnel and other departments requiring frequent interaction to deliver quality services.  Participates in teaching patients, family members, and/or significant others as appropriate to the physician-ordered plan of care.  Refers to other members of the health care team to meet specific learning needs of the patient as appropriate.  Notifies Case Manager of changes in patient condition as necessary.  Addresses patients, families, and co-workers in a responsive and respectful manner.  Always provides safe care which is in concert within physician orders, agency policy and established nursing care standards. Participates in communication and interdisciplinary activities to promote maximum continuity and provide optimal care.  Attends and contributes to both formal and informal patient care planning conferences.  Completes all forms concerning patient visits and mileage information according to agency policy.  Maintains and respects patient confidentiality.  Understands the patient's medical record is a legal document and maintains accurate documentation and respects confidentiality of same.  Improves patient care through participation in continuing education and in-service.  Comply with Accentra Hospice’s emergency preparedness plan.  Provide accurate, timely, complete, and acceptable documentation:Accurate – Reflecting good nursing judgment and as - Documentation to be within 48 hours of the - All notes completed in totality, with a legible and understandable narrative. All admission documents completed and - Meeting acceptable Medicare guidelines as explained and documented in accordance with Accentra Hospice Policies and Procedures.  Exhibits sound judgment and technical skills necessary to perform assigned nursing treatments and procedures.  Effectively communicates with team members to keep them informed of changes in patient condition and needs.  Notify Case Manager and PCC immediately whenever unable to complete patient care assignment.  Demonstrates awareness through safe practice of established nursing policies and procedures related to every technical procedure performed.  Demonstrates knowledge of underlying disease processes, surgical procedures or injuries of assigned patient.  Displays adequate knowledge of safe administration of medications and the instruction of patients.  Implements care by setting priorities based on the patient care plan and immediate patient need.  Evaluates the response of patients to nursing interventions and revises care plans with Case Manager as necessary.  Performs all treatments and procedures with consideration for the patient's condition, safety, comfort and privacy.  Notes normal and abnormal conditions and reports accurately.  Demonstrates awareness of infection control policies and procedures and the practice of asepsis in the performance of all personal patient care and treatment procedures.  Observes universal precautions when caring for all patients.  Practices isolation techniques according to established agency policies and procedures.  Complies with OSHA regulations and recommendations.  Practices good body mechanics when lifting or transferring patients or equipment.

Functional Abilities  

Contact with a variety of housing conditions required. Must be able to maintain concentration during frequent interruptions.  Excellent verbal and written communication skills.  Must be flexible to adjust to changing tasks frequently. Light to moderate degrees of stress incurred in daily employment.  Able to communicate verbally and in writing to the extent required by the position.  Able to physically perform the duties required by the position.  Able to travel. 

Physical Requirements 

Moderate physical activity required, including possible lifting, up to 50lbs and turning of adult patients.  Good color vision essential to determine subtle changes in patient's skin color and to rapidly recognize medication and color-coded supplies. Requires good hearing to assess situations quickly and respond appropriately.
