Job Summary:
We are seeking a compassionate and skilled Hospice LPN Care Specialist to join our team. As a Hospice LPN Care Specialist, you will be responsible for providing high-quality patient care, working closely with our interdisciplinary team to develop and implement individualized care plans, and maintaining accurate and timely documentation.
Responsibilities:
- Provides direct patient care, including administering medications, performing wound care, and providing emotional support to patients and their families.
- Collaborates with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conducts thorough assessments of patients' physical and emotional needs, documenting all findings and interventions in a clear and concise manner.
- Maintains accurate and timely documentation, including progress notes, medication records, and other relevant information.
- Communicates effectively with patients, families, and healthcare team members to ensure seamless coordination of care.
- Participates in ongoing education and training to stay current with best practices and industry developments.
- Contributes to a positive and supportive work environment by demonstrating a commitment to teamwork, empathy, and excellent customer service.
Requirements:
- Certified Licensed Practical Nurse (LPN) license in the State of Oklahoma.
- Previous hospice nursing experience preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
- Ability to maintain confidentiality and handle sensitive information.
- Basic computer skills and proficiency in electronic medical records.
