Job Summary

As a Home Health Speech Language Pathologist, you will be responsible for providing high-quality speech and language services to patients in their homes, promoting independence and improving communication skills.

Key Responsibilities
  • Develop and implement individualized speech therapy plans for patients with speech and language disorders.
  • Conduct assessments and evaluations to identify patient needs and goals.
  • Provide education and training to patients, families, and caregivers on communication strategies and techniques.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Master's degree in Speech Language Pathology or related field.
  • Active and unencumbered Speech Language Pathologist license in state(s) of employment.
  • Minimum of one (1) year of experience in a home health setting.
