Job Summary

The Home Health Speech Language Pathologist is responsible for providing speech and language services to patients in their homes. This role requires a strong understanding of speech and language disorders and the ability to develop and implement effective treatment plans. The ideal candidate will have a passion for working with patients and families in a home health setting and will be able to communicate effectively with healthcare professionals and other stakeholders.

Key Responsibilities
  • Assess and evaluate patients' speech and language abilities and develop individualized treatment plans.
  • Provide speech and language therapy services to patients in their homes, including assessment, treatment, and education.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient progress and treatment plans in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans.
  • Stay up-to-date with industry developments and best practices in speech and language therapy.

Requirements
  • Bachelor's degree in Speech Language Pathology required; Master's degree preferred.
  • Active and unencumbered Speech Language Pathologist license in state(s) of employment required.
  • Minimum of one (1) year of experience in a home health setting required.

  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to adapt to changing situations and priorities.
  • Strong analytical and problem-solving skills.

Work Environment
  • Regular standing, walking, and manual dexterity are fundamental.
  • Ability to lift and move objects up to 50 pounds.
  • Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
