Clinical Care Coordinator-BH LCSW

Job Summary:
We are seeking a highly skilled Clinical Care Coordinator-BH LCSW to join our team at Nuvance Health Med Practice CT. As a Clinical Care Coordinator-BH LCSW, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare settings.
Key Responsibilities:- Coordinate multiple aspects of patient care to ensure timely and effective transitions between healthcare settings.
- Assess patient clinical readiness for transition and inform the healthcare team of available resources.
- Develop and implement patient care plans in collaboration with the healthcare team.
- Provide education to patients and families regarding coverage, care options, and community resources.
- Collaborate with the healthcare team to identify and address delays in patient care.
- Participate in the development and improvement of clinical initiatives focused on reducing LOS, denials, and re-admissions.
- Act as an intermediary between patients, physicians, and community providers to ensure accurate healthcare information and proper treatment plans.
- Initiate and coordinate patient care conferences as required.
- Provide clinical social work interventions, including counseling, bereavement, and crisis intervention.
- Review admissions and continued stays for assigned patients to determine certification to the assigned level of care.
- Broad-based clinical background and hospital experience.
- Excellent oral and written communication skills.
- Basic computer skills.
- Ability to prioritize daily workload and utilize tools dedicated to the position/function.
- Ability to deal with a high degree of public and patient contact and interaction.
This position requires a sedentary/light effort and may exert up to 10 lbs. force. The physical environment is generally pleasant working conditions.
