Clinical Care Coordinator-BH LCSW Job Description

Job Summary:

We are seeking a highly skilled Clinical Care Coordinator-BH LCSW to join our team at Nuvance Health Med Practice CT. As a Clinical Care Coordinator-BH LCSW, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities:
  • Coordinate multiple aspects of patient care to ensure timely and effective transitions between healthcare settings.
  • Assess patient clinical readiness for transition and inform the healthcare team of available resources.
  • Develop and implement patient care plans in collaboration with the healthcare team.
  • Provide education to patients and families regarding coverage, care options, and community resources.
  • Collaborate with the healthcare team to identify and address delays in patient care.
  • Participate in the development and improvement of clinical initiatives focused on reducing LOS, denials, and re-admissions.
  • Act as an intermediary between patients, physicians, and community providers to ensure accurate healthcare information and proper treatment plans.
  • Initiate and coordinate patient care conferences as required.
  • Provide clinical social work interventions, including counseling, bereavement, and crisis intervention.
  • Review admissions and continued stays for assigned patients to determine certification to the assigned level of care.
Requirements:
  • Broad-based clinical background and hospital experience.
  • Excellent oral and written communication skills.
  • Basic computer skills.
  • Ability to prioritize daily workload and utilize tools dedicated to the position/function.
  • Ability to deal with a high degree of public and patient contact and interaction.
Working Conditions:

This position requires a sedentary/light effort and may exert up to 10 lbs. force. The physical environment is generally pleasant working conditions.
