Job Description

At Creative Community Services, Inc., we are seeking a highly skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing exceptional support to individuals with developmental disabilities.

Key Responsibilities:

  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Provide ongoing support and advocacy to ensure the best possible outcomes for our clients.
  • Collaborate with our team to ensure seamless care coordination and communication.
  • Document all interactions and progress in a timely and accurate manner.
  • Participate in regular supervision and training to enhance your skills and knowledge.

Requirements:

  • Bachelor's degree in a human services field or related field.
  • Experience working with individuals with developmental disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
