Job Overview:
- The Patient Care Coordinator is responsible for managing the coordination of clinical services for patients transitioning from hospital to home or other care settings.
- This role involves promoting, educating, and coordinating services during patient transfers to home healthcare or hospice care.
Qualifications:
- A Bachelor’s degree in nursing is preferred.
- Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
- A minimum of 2 years of experience in home health or hospice settings is preferred.
- Knowledge of community resources is advantageous.
Licensure and Certification:
- Must hold a valid Registered Nurse license in the State of South Dakota and/or meet Multi-state licensure requirements.
- Licensure in the State of Minnesota may be necessary based on service area requirements.
- Integrated Care Management Model Certification is preferred.
- A valid driver’s license and insurability under Avera Health’s automobile liability insurance are required.
Work Schedule:
Monday to Friday, with occasional holidays; 16 hours every two weeks.
Compensation:
The hourly wage for this position is competitive, starting at $44.25 per hour, depending on experience.
