JOB SUMMARY AND QUALIFICATIONS:
The primary responsibility of the Targeted Case Manager is to deliver case management services to families engaged in the Family Recovery Program.
The Case Manager will assist individuals and families in accessing necessary housing, medical, social, educational, and other identified services.
WHAT YOU SHOULD HAVE FOR THIS ROLE:A Bachelor's degree in a behavioral science field (including psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree).
A minimum of one (1) year of full-time experience working directly with adolescents or adults in a human service environment; preferably in the Substance Use field; (2) successful completion of case management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) and completion of recertification requirements approved by KBHDID every three years.
(3) knowledge and skills in case management, crisis counseling, community resources, advocacy, problem-solving, critical thinking, communication, mental health, substance use, and documentation.
A Master's degree in a behavioral science is preferred.RESPONSIBILITIES:
1 Provide case management services tailored to the specific needs of individuals and families. Some services may be delivered in the home of the individual or family.
2 Conduct thorough assessments and reassessments to identify the need for housing, medical, social, educational, and other services.
3 Create a personalized case plan for each individual and/or family based on the assessment. The plan will outline the goals and activities addressing housing, medical, social, educational, and other identified services.4 Refer individuals and/or families to appropriate services based on the assessment. This may include scheduling appointments and ensuring transportation is arranged for those appointments.
5 Monitor and follow up on the goals and activities established in the case plan. This may involve follow-up with the individual, family members, and other service providers. Update the case plan as necessary.
6 Identify community resources and cultivate collaborative relationships with other social service providers.
7 Accurately represent agency programs, policies, and procedures within the community and with other social service agencies.
8 Maintain precise case records, including proper documentation of all interactions with clients.
9 Ensure all documentation adheres to KY Medicaid and accreditation standards.
10 Conduct Life Skills classes for identified individuals in need of this service.
11 Participate in weekly case conferencing and provide updates on all assigned individuals and/or families.
12 Engage in bi-monthly supervision with the TCM billing Supervisor.
13 Attend monthly staff meetings and lead training sessions/discussions as assigned.
14 Complete the annually required 10-hour SUD training to maintain specialty certifications.
15 Uphold certification requirements for TCM as per Kentucky Board regulations.
16 Assist with special projects as assigned.
17 Support reporting requirements as assigned.
