Job Summary

We are seeking a skilled Clinic Coordinator to join our team at UnitedHealthcare. As a Clinic Coordinator, you will play a critical role in supporting the health and well-being of our members, particularly those with complex health conditions.

Key Responsibilities
  • Provide evidence-based education and support to promote healthy pregnancy, delivery, and postpartum outcomes for mothers and babies.
  • Engage members in managing their own health and healthcare through proactive outreach and communication.
  • Collaborate with healthcare providers, hospitals, and community organizations to ensure seamless care coordination.
  • Travel within the assigned region to visit members in their homes and other settings, including community centers, hospitals, and providers' offices.
  • Work closely with a team of nurses to support clinical decision-making and provide comprehensive care coordination.
Requirements
  • Bachelor's degree in health, human, social work, or education services, or equivalent experience.
  • 1+ years of qualifying experience in care coordination, particularly with complex health conditions or maternal and child health care.
  • Ability to travel up to 50% of the time and work in a field-based role with a home-based office.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
