Position Overview:
As a Clinical Coordinator, you will leverage your expertise in working with individuals facing developmental disabilities or mental health challenges. This role is integral to advancing our mission of delivering exceptional service through effective case management and counseling.

Key Responsibilities:

  • Deliver comprehensive case management and counseling services to a designated caseload, ensuring thorough assessment and collaboration with treatment teams, funding sources, and stakeholders.
  • Maintain accurate and up-to-date records in compliance with corporate, contractual, and regulatory standards.
  • Provide continuous monitoring and guidance to mentors on the implementation of service plans, including pre-service introductions and ongoing education.
  • Conduct home visits and facilitate group meetings for mentors and families as necessary.
  • Support emergency coverage and crisis intervention efforts, participating in a rotational on-call system.
  • Assist with legal, licensing, regulatory, and certification processes as required.
  • Engage in activities that promote the development of a robust network of mentors.

Qualifications:

  • Master's degree in human services; Bachelor's or Master's in Social Work as mandated by state regulations.
  • A minimum of two years of relevant experience with individuals facing developmental disabilities or mental health issues.
  • Detail-oriented and self-motivated, with the ability to manage multiple tasks effectively.
  • Proven ability to inspire teams and achieve organizational goals.
  • Valid driver's license in good standing and access to a registered vehicle with proof of insurance for transportation purposes.
  • Appropriate state licensing (LBSW, LMSW, LCSW, or LCSW-C) as required.
